Directory of all application forms used at Pakistan embassies and high commissions. Most forms are available as PDF downloads from individual embassy websites or can be collected in person.

Note: Form availability and format may vary between embassies. Always check your specific embassy's website for the latest version of forms. Some embassies have moved to online application systems, replacing paper forms.

Passport Forms

Tips for Completing Forms

  • Use block capitals (CAPITAL LETTERS) unless otherwise instructed
  • Use black or blue ink only
  • Do not use correction fluid (Tipp-Ex) — if you make a mistake, start a new form
  • Sign in the designated boxes only
  • Ensure your name matches exactly across all documents (passport, NICOP, forms)
  • Bring photocopies of completed forms for your records
  • Some embassies accept printed (typed) forms — check with your embassy
